    Unfortunately, there is a small mistake regarding reference 7 ("Pre-hospital airway management: guidelines from a task force from the Scandinavian Society for Anaesthesiology and Intensive Care Medicine."), which is not published in Anesth Analg but rather in Acta Anaesthesiol Scand.

    Published in the mean time, the correct reference is:
